填表说明：
1. 不同品种的计量器具分别填写申请书。
申请书一式四份（申请单位、申请受理人、型式试验单位、OIML中国秘书处各一份）。
3. 填写整齐清楚。模糊和无印章（国内）、无签字（国外）无效。
4. 技术规格主要包括：测量范围、准确度等级、仪器分辨率、仪器适用范围、仪器的体积、重量等。
审查意见和处理决定由接受申请的单位填写。内容包括是否接受申请，是否需要做试验以及做试验的单位。
6. 申请者除提交申请书和样机外，还需提供下述文件：
A. 技术说明书
产品检验方法
C. 型式批准证书及试验报告（如果有）
7. 除产品简要说明外此表用中英两种文字填写。
Note:
1. Different categories of measuring instruments should be applied separately.
2. Four copies of an application (applicant, representative of applicant, testing body and Office for OIML Affairs has one copy separately).
3. Application should be tidy and clear. Ambiguous and without seal (domestic) or sign (foreign country) are invalid.
4. Specification (measuring range, accuracy class, resolution, applicable scope, weigh and volume: L.W.H.)
5. Checking conclusion and decision is filled by body accepted application: In checking conclusion and decision, body accepted should made it clear whether application to be accepted and any tests to be required and laboratories responsible testing.
6. Except for application and samples, documents are required:
A. Technical manual
B. Testing methods
C. Type approval certificate and test report (if applicable).
7. Filling this application both in Chinese and English except brief description.
